Appointments
If you would like to make an appointment, please contact our office by phone or email. We have implemented call forwarding to better accommodate emergency calls during our off hours.  For your convenience, please feel free to print and fill out our intake forms to bring with you on your first appointment with us.  See Patient Intake Forms Tab under Forms Tab.

 

Financial
We are happy to offer a new discount program to keep our rates affordable for those without insurance or under-insured patients through the ChiroHealthUSA Network. You can apply through our office, and it goes into effect immediately on your first visit with us, giving you a 50% discount on all of our services.

We will submit insurance claims for those with insurance benefits, as well as medicare, personal injury, and workman's compensation claims. We will do everything we can to help you afford the treatment you need and want. Forms of payment accepted by the office are check, cash, or any major credit card.

Cancellation
If you are unable to keep an appointment, we ask that you kindly provide us with at least 24 hours notice. We ask for this advance notice so that we can offer this appointment to another patient.
